Partnering with Support Coordinators to Achieve Real Outcomes
"Without their positive support, I can honestly say I would not be where I am today—back home with my family, where I belong." – George P.

How We Work Together
Open and Transparent Communication
We make it a priority to maintain open, proactive, and transparent communication with Support Coordinators. From the first point of enquiry through onboarding and throughout service delivery, our team ensures Support Coordinators are informed, supported, and engaged every step of the way.
Collaborative Planning & Problem Solving
Our approach is centred on co-designing supports that align with participant goals. We work closely with Support Coordinators, participants, and their networks to ensure support plans are tailored, realistic, and flexible to changing needs.
When complex or urgent situations arise, we work as a team — bringing together expertise, lived experience, and creative problem-solving to support the best possible outcomes.
Quality Reporting & Outcomes Focus
Our transparent reporting and proactive communication give Support Coordinators clarity and confidence, helping them advocate effectively for participants while ensuring services remain focused on real outcomes, not just hours delivered.
Real Story: Supporting George P. to Access the Right Support
When George P. found himself in a psychiatric unit and unable to return home, his Support Coordinator Don reached out to us. Together, we recognized that George needed more than just accommodation – he needed comprehensive support to rebuild his life and return to his family.
Working alongside Don, we identified that George was critically underfunded, receiving just 2 hours of support per week. This was far below what he needed to meet his daily living requirements and ensure his wellbeing. Through collaborative advocacy and evidence gathering, we built a strong case for an escalated plan review.
The result? George's NDIS plan was increased from 2 to 20 hours of support per week – a life-changing outcome that has enabled him to return home to his family. But don't just take our word for it. Here's what George has to say about his experience:
My name is George P. I am a single, middle-aged man with close family ties. Due to circumstances beyond my control, I found myself in a psychiatric unit, unable to return home. I was lost, alone, and suddenly homeless. But to my surprise, there were still compassionate individuals who put my needs first and listened to my concerns above all else. Some of the people who cared for me during my stay at Christine Court Assisted Living became like a second family to me—even to this day. The rooms were spacious and comfortable, each with its own ensuite. Meals were generous, served three times a day on a rotating menu, and my medication was always given at the prescribed time. But what meant the most to me was the staff's willingness to offer advice and support, especially in my darkest hours—or simply to have a chat when I needed it. It was clear that these were people genuinely invested in the work they were doing, not just as a job, but as a calling to help others. Without their positive support, I can honestly say I would not be where I am today—back home with my family, where I belong. I would absolutely recommend Christine Court Assisted Living, or their partner company INREACH Supports, to anyone who needs a little—or a lot—of assistance with day-to-day living. These people truly care.
